How do I find a GP/change my GP?

Contact Birmingham Health Authority on (0121) 695 2222 and staff
will send you a list of GPs in your area. To help you decide which
GP would be best for you, ask for a copy of the practice leaflet –
this gives information on the range of services provided there. It
might also be useful for you to consider the following:

Would you prefer a male or female doctor?
|
Do you have any special health needs? (Some GPs have a particular
interest in some illnesses, such as diabetes or asthma)
|
Would you prefer a small or large practice (one GP or several GPs
within the same practice)?
|
|
Telephone or visit the practice and ask to be registered. GPs can
refuse to accept patients without giving a reason; however, this is
usually because their list is already full. If the GP accepts you, the
practice will arrange for the transfer of your medical records from
your previous doctor. Tell the Practice Manager if you feel the need
to request an urgent transfer of your records, otherwise this can take
up to 3 months.

How do I find an NHS dentist?
|
|
Telephone Birmingham Health Authority on (0121) 695 2222 and
ask to be sent a list of dentists in your area. Check with the dentist
which services they provide on the NHS.

NHS dental treatment charges
|
|
You will be exempt from NHS dental treatment charges if you are in
one of the following categories:
|
|
you are under 18, or are 19 and in full-time education
|
you are pregnant when the treatment starts, or have had a baby in the
12 months before your treatment starts (this includes women who
have had a still birth)
|
you or your partner receives income support, job seekers allowance,
family credit, disability work allowance or either of you holds an
AG2 certificate
|
|
If you think you qualify for an exemption, ask at your dentists, post
office or pharmacists for an AG1 booklet – this gives full details of
all exemptions
|
|
All NHS dental treatments have a fixed cost, and unless you are
exempt from treatment you must pay 80% of this. Before you start
any NHS treatment, ask your dentist for a FP17 DC form. Your
dentist should go through your treatment plan with you and tell you
how much your treatment will cost.
